This page serves as a singular landing point where CVRC SCs and Vendored Providers can locate the resources you want to support you throughout the liaison monitoring process!
Timeline:
- During the month of April 2023 CVRC SC Liaisons will be reaching out to Providers to schedule the Liaison Monitoring visit.
- Liaison Monitoring visits will occur from mid-April thru June 16, 2023.
- Liaison’s have till end of day on June 16, 2023 to complete all paperwork and upload into DocuSign.
Tasks for the CVRC SC Liaison regarding Liaison Monitoring:
• Initial phone call to schedule meeting
• Walk the Provider through the Liaison Monitoring form as you review their documents. The CVRC Liaison is responsible for filling out the form and submitting it.
• Verify documents for accuracy – whether that is the weekly staff schedule to checking fire alarms
• If Liaison is unsure about documentation, please reach out to your program manager
• Take time and do not rush the meeting
Tasks for CVRC Care Provider:
• Agree on a time, where there is limited distraction
• Have documentation prepared and accessible
• Liaison will need to review documents thoroughly for accuracy
• Take time and do not rush the meeting
