DDS Self-Advocate and Family Survey

COVID-19 has impacted all of us, in different ways. Your experience can help the Department of Developmental Services (DDS) – and all of the organizations meeting the needs of self-advocates and families – better understand the pandemic’s effects on people with developmental disabilities.

DDS is asking for your help in distributing the survey below to self-advocates and families. The survey will inform us, as a system, how COVID-19 has impacted our community and changed services and identify what is important to self-advocates and families

The survey includes fifteen questions and is available in 10 languages.
